CONSULTATION: Non-Crisis Mode
This type of consultation is utilized whereby your loved one is showing noticeable signs of cognitive and/or physical impairment. For the time being, however, your loved one will remain at home.
This form of consultation includes:
a. review of their Will (or Living Trust), Power of Attorney, Living Will and Healthcare Proxy.
The purpose of said review is to determine if such documents are in the “ready-ready” mode in light of their changing health circumstances.
BEING IN THE “READY-READY” MODE:
Allows someone in the family to utilize the exceptions to the NJ Medicaid 5-yr. lookback rule should your loved one enter a Nursing Home or Assisted Living setting.
